BILLING & INSURANCE

The information provided on this page is for clients interested in or currently participating in individual therapy with me. 

Payment

Please be prepared to make payment at the time services are rendered (e.g., copay or co-insurance for clients using insurance and full payment for self-pay clients). 

Cancellation Policy

Please remember to cancel or reschedule therapy appointments at least 24 hours in advance to allow me time to offer the space to someone else. For cancellations made without at least 24 hours of notice, there is a $150 cancellation fee. This policy is meant to reflect our mutual respect for one another's time and the work we are doing together.

Insurance

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully to be sure that you understand the answers to the following questions:

  • Do I have mental health insurance benefits?

  • What is my deductible and has it been met?

  • How many sessions (per year) does my health insurance cover?

  • What is the coverage amount per therapy session?

  • How much does my insurance pay for an out-of-network provider?

  • Is approval required from my primary care physician?

  • What is my co-pay or co-insurance for each visit?

 

Insurance Billing – In-network provider: If you are covered by insurance with one of the providers with whom I am in-network, I will submit insurance claims with those companies on your behalf.  You are expected to make any co-pay at the time of our session.  

I am an in-network provider for the following insurance companies:

  • Aetna

  • First Choice Health

  • Moda

  • PacificSource

  • PacificSource Community Solutions

  • Regence BlueCross BlueShield of Oregon

  • Trillium Behavioral Health

Insurance Billing – Out-of-network provider:  If you would like to bill insurance with a company with whom I am not in-network, I can provide you with a monthly invoice so you can submit for reimbursement.  In that situation, you are expected to make payment at the beginning of each session and work with your insurance company for reimbursement.
